Cynosure Health Improvement Advisor, Barb DeBaun, RN, MSN, CIC, facilitated an action-oriented session at the MBQIP in-person meeting on January 28, 2020 in West Virginia. The ice was broken with an exercise designed to showcase the power of teamwork when solving a problem. It was attended by Quality Leaders, Chief Nursing Officers, Case Managers, Social Workers, and Staff Nurses. The focus of discussion was on improving MBQIP measures, especially patient’s perceptions of care transitions. All attendees established improvement goals and a small test of change. Learnings from these tests will be shared at our summer convening in Bridgeport, West Virginia.

Activities included a facilitated discussion of MBQIP measures and the ‘state of the state’. An interactive ‘Broken Squares’ exercise was performed to demonstrate problem solving and teamwork. The focus was on improving care transition measures, established improvement expectations, and next steps.
