Florida’s Statewide Readmissions Response

PROJECT AIMS:

In response to wide variation in readmissions performance, the Florida Readmissions Collaborative was started as the first program to address readmissions at scale statewide.

APPROACH/PARTNERS:

Cynosure Health’s partners included the Florida Hospital Association, more than 100 hospital participants, Florida’s Agency for Health Care Administration, the Florida Orthopedics Society, the INTERACT program, skilled nursing facilities, and local community-based organizations.
